Medication at School

If your student needs medication while at school, you will need to fill out one or more of the following medical forms:
Authorization to Give Medication (English) (Spanish)
- For medication (prescription or over the counter) to be kept in the school clinic
- Over the counter medication needs to be age appropriate
Authorization to Carry Over the Counter Medication (English) (Spanish)
- Middle school students may carry some medications: Tylenol, Acetaminophen, Motrin,
Advil, Ibuprofen, Midol, Aspirin, antacids, cough and throat lozenges, and oral antihistamines.
Any medication not listed must be kept in the clinic
Authorization to Carry Prescription Medication (English) (Spanish)
- For prescription inhalers, epinephrine auto injectors, or diabetic medication only
- Form requires physician signature
